Analysis of Lesion Distribution and Characteristics on Breast Ultrasonography in Asymptomatic Women Aged 20s and 30s
This study analyze the distribution, Breast Imaging Reporting and Data System (BI-RADS) categories, and morphological characteristics of lesions detected on screening breast ultrasonography in asymptomatic women aged 20s and 30s. A retrospective analysis was performed on 121 women examined at a single institution between January 2023 and June 2025. The overall lesion detection rate was 54.3%, with BI-RADS 2 accounting for 66.9%. Lesions were most frequently detected in the upper outer quadrant of both breasts, accounting for 24.8% on the right and 19.0% on the left. The distribution of lesion locations differed significantly according to BI-RADS category (Fisher-Freeman-Halton exact test, p=0.028). In addition, as the BI-RADS category increased, the proportions of non-circumscribed margin, non-parallel orientation, and posterior acoustic shadowing increased significantly (χ²=36.94, p<0.001). These findings suggest that breast ultrasonography may aid lesion detection and morphological evaluation in asymptomatic young women. The results may also serve as baseline data for establishing follow-up and management planning.

본 연구는 20~30대 무증상 젊은 여성을 대상으로 검진 유방초음파에서 나타나는 병변의 분포, 유방영상 판독 및 데이터 시스템(Breast Imaging Reporting and Data System, BI-RADS) 범주 및 형태학적 특성을 분석하고자 수행되었다. 2023년 1월부터 2025년 6월까지 단일기관에서 검진 목적으로 유방초음파를 시행한 121명을 대상으로 후향적 분석을 시행하였다. 그 결과 전체 병변 발견율은 54.3%였으며, BI-RADS 범주별로는 BI-RADS 2가 66.9%로 가장 많은 비중을 차지하였다. 병변의 위치는 양측 유방 모두 상외사분면에서 가장 빈번하게 관찰되었으며(우상외사분면 24.8%, 좌상외사분면 19%), BI-RADS 범주에 따른 위치 분포는 통계적으로 유의한 차이를 보였다(Fisher-Freeman-Halton exact test, p=0.039). 또한 BI-RADS 등급이 높아질수록 비정형경계, 비평행방향성, 후방음향음영의 비율이 유의하게 증가하였다(χ²=36.94, p<0.001). 이러한 결과는 무증상 젊은 여성에서 유방초음파가 병변의 조기 발견과 형태학적 특성 평가에 유용하게 활용될 수 있음을 시사하며, 향후 추적 관찰 및 관리 계획 수립을 위한 기초 자료로 활용될 것으로 기대된다.
