무릎관절 전치환술 환자에서 모바일 기반 원격재활이 신체기능 및 심리적 요인에 미치는 영향 : 전향적 비무작위 비교 연구
Effects of Mobile-Based Telerehabilitation on Physical Function and Psychological Outcomes in Patients Undergoing Total Knee Arthroplasty : A Prospective Non-Randomized Controlled Study
The purpose of this study was to evaluate the effects of mobile-based telerehabilitation on physical function and psychological factors in patients undergoing total knee arthroplasty(TKA). Thirty-one patients participated in this prospective non-randomized controlled study. Patients were allocated to an experimental group(n=16) and a control group(n=15). Both groups received conventional rehabilitation, while the experimental group additionally participated in a mobile-based telerehabilitation program for 2 weeks after surgery. Physical function was assessed using range of motion(ROM) and WOMAC scores, and psychological status was evaluated using K-HADS-A. The experimental group showed significant improvements in active flexion(p<.014), active extension(p<.017), passive flexion(p<.047), WOMAC pain scores(p<.001), and activities of daily living scores(p<.016) compared with the control group. Although no significant between-group difference was found in anxiety levels, the experimental group showed a greater tendency toward anxiety reduction. These findings suggest that mobile-based telerehabilitation may be an effective intervention for improving early functional recovery and pain reduction following TKA and may serve as a clinically useful adjunct to conventional face-to-face rehabilitation. In addition, this study demonstrated the potential clinical applicability of a mobile platform-based video-assisted rehabilitation program without the need for expensive equipment.

본 연구의 목적은 무릎관절 전치환술 환자(TKA)를 대상으로 모바일 기반 원격재활이 신체 기능 및 심리적 요인에 미치는 효과를 검증하는 것이다. 환자 31명을 실험군(16명)과 대조군(15명)으로 구분하여 수술 후 2주간 분석한 결과, 실험군은 능동 굴곡(p<.014), 능동 신전(p<.017), 수동 굴곡(p<.047) 등 모든 ROM 지표에서 유의한 향상을 보였다. WOMAC 지수 내 통증(p<.001)과 일상생활 수행능력(p<.016)도 크게 개선되었다. 그러나 불안 점수는 군 간 차이가 없었다(p>.264). 결과적으로 모바일 기반 원격재활은 TKA 환자의 초기 기능 회복과 통증 완화에 효과적이며, 대면 재활을 보완할 유용한 중재임을 시사한다. 또한 본 연구는 별도의 고가 장비 없이 모바일 플랫폼 기반의 영상 보조 재활만으로도 임상적으로 유의한 기능 회복 효과를 확인하였다는 점에서 실제 임상 현장 적용 가능성을 제시하였다. 다만 심리적 효과는 군 간 유의한 차이가 확인되지 않았으므로, 향후 충분한 표본을 확보한 장기 추적 연구를 통해 모바일 기반 원격재활의 심리적 효과와 지속 효과를 재검증할 필요가 있다.

Ever since next generation convergence technology became one of the most important industries in the nation, computing professionals have encountered a growing number of challenges. Along with scholars and colleagues in related fields, they have gathered in avariety of forums and meetings over the last few decades to share their knowledge, experiences and the outcome of their research.
