This study analyzes the neurobiological mechanisms and psychological dysfunctions shared by sexual and drug addictions and proposes a multidisciplinary integrated treatment model to address them. Sexual addiction is a complex pathology combining the loss of prefrontal cortex control due to disrupted dopamine reward circuits with traumatic attachment injuries and distorted personality structures. In particular, compulsive sexual behavior combined with substance use (Chemsex), such as methamphetamine, abnormally raises the brain's reward threshold, leading to sexual anhedonia and existential crises, which in turn inflicts severe “betrayal trauma” on partners. To facilitate recovery from these complex addictions, this study suggests a three-dimensional integrated approach. First, for neurobiological rewiring, it aims to restore cognitive control functions using TLR4-targeted vaccines and neuromodulation (tDCS), while reconstructing physical functions through Low-Intensity Extracorporeal Shockwave Therapy (Li-ESWT). Second, through the introduction of digital technology, it establishes an "External Co-cognition" system that predicts high-risk situations in real-time and implements Ecological Momentary Intervention (EMI) using “Digital Sensing” and “Digital Phenotyping” via wearable devices. Third, as psychological and environmental interventions, it applies sexual cognition restructuring through Sensate Focus and a self-control framework based on Harm Reduction using the mobile application “Budd.” In conclusion, addiction treatment must move beyond mere cessation of behavior to become a holistic process of rebuilding shattered attachment bonds and healing both the brain and mind through the integration of digital and medical technologies. This multidisciplinary model, grounded in compassionate self-love, will serve as a key mechanism for restoring the existential rights of addicts and enabling their full reintegration into society.

본 연구는 성중독과 마약중독이 공유하는 신경생물학적 메커니즘과 심리적 역기능을 분석하고, 이를 해결하기 위한 다학제적인 통합치료모델을 제시하고자 한다. 성중독은 도파민 보상회로의 교란으로 인한 전전두엽의 통제력 상실이라는 신경생물학적 특성과 외상적 애착 손상 및 왜곡된 성격구조가 결합된 복합적 병리 현상이다. 특히 필로폰 등 약물사용과 결합된강박적 성행동인 “캠섹(Chemsex)”은 뇌의 보상역치를 비정상적으로 높여 성적 무쾌감과 실존적 위기를 초래하며, 이는 배우자에게 심각한 “배신외상(Betrayal Trauma)”을 입히는 원인이 된다. 이러한 복합중독의 회복을 위해 본 연구는 세 가지 차원의 통합적 접근을 제안한다. 첫째, 신경생물학적 재배선을 위해 톨유사수용체 4(TLR4) 표적 백신과 경두개직류자극술(tDCS)을 활용하여 뇌의 인지제어기능을 개선하고, 저강도체외충격파치료(Li-ESWT)를통해 신체적 기능을 재건한다. 둘째, 디지털기술의 도입으로, 웨어러블 기기를 활용한 “디지털센싱”과 “디지털표현형” 분석을 통해 고위험 상황을 실시간으로 예측하고 생태학적 순간개입(EMI)을 실행하는 “외부보조인지(External Co-cognition)” 체계를 구축한다. 셋째, 심리적·환경적 개입으로서 감각집중훈련(Sensate Focus)을 통한 성인지 재구조화와 모바일 앱“버드(Budd)”를 활용한 위험감소(Harm Reduction) 기반의 자기조절 프레임워크를 적용한다. 결론적으로 중독치료는 단순한 행위 중단을 넘어, 파괴된 애착 유대감을 재건하고 디지털 및 의료기술을 결합하여 뇌와 마음을 동시에 치유하는 전인적인 과정이어야 한다. 자비로운 자기사랑(Self-love)을 바탕으로 한 이러한 다학제적인 통합치료모델은 중독자의 실존적권리를 회복하고 온전한 사회복귀를 가능케 하는 핵심 기제가 될 것이다.
