Purpose: This study aimed to analyze longitudinal changes in the perceptions of emergency medical technicians (EMTs) regarding the introduction of a licensure system and identify the structural causes of occupational conflicts concerning implementation methods (EMT-paramedic priority vs. simultaneous implementation for EMT-paramedics and EMT-basics). Methods: A descriptive cross-sectional study was conducted using survey data from 10,787 EMTs (6,611 in 2020 and 4,176 in 2023). Data were analyzed using chi-square tests with adjusted residuals and a binomial logistic regression model to determine the factors influencing the preferred implementation method. Results: While 97.4% of the respondents supported the licensure system, significant differences emerged regarding the implementation methods. EMT-paramedics strongly preferred priority implementation. Conversely, EMT-Basics, those with more clinical experience, and public-sector workers significantly preferred simultaneous implementation. Controlling for general characteristics, the logistic regression indicated that EMT-Basics were 26.48 times more likely to prefer simultaneous implementation compared to the reference group of EMT-Paramedics (aOR=26.48, p<.001). Conclusion: Despite a strong consensus on the necessity of licensure, severe occupational conflicts exist regarding the implementation methods. Resolving the dualized training system and securing educational homogeneity are essential prerequisites for a successful transition. However, as specific reform measures lack social consensus, further discussions and follow-up studies are required to develop a detailed roadmap.
