This narrative review aimed to elucidate the structural and systemic determinants underlying the high burden of chronic pain and musculoskeletal disorders, as well as the limited access to interventional pain management in rural areas. A comprehensive literature search was conducted with an emphasis on the epidemiology of chronic pain, rural-urban health disparities, agricultural work environments, and healthcare accessibility. A total of fourteen studies published since 2000, including nine from Korea and five from other countries, that explicitly examined rural populations or agricultural workers were included. Across studies, rural workers consistently demonstrated a disproportionately high prevalence of pain, particularly in the low back, shoulder, and knee. Repetitive and high-intensity manual labor, non-ergonomic postures, aging, and socioeconomic vulnerability were frequently identified as key contributors to pain onset and chronification. In addition, the restricted availability of medical specialists, limited interventional pain and rehabilitation services, long travel distances, financial constraints, and low health literacy were reported to hinder timely diagnosis and treatment, thereby exacerbating chronic pain and functional decline among rural residents. These findings suggest that chronic pain in rural settings is a complex, multifactorial condition arising from the interaction of occupational load, demographic aging, social disadvantage, and healthcare shortages. Comprehensive, multi-level strategies, including workplace modification, musculoskeletal prevention programs, strengthening local medical infrastructure, and the use of telemedicine and mobile clinics, are required to mitigate these structural disparities.

본 연구는 농촌 지역의 만성통증 부담과 통증중재치료 접근성의 구조적 격차를 규명하기 위해 수행된 서술적 문 헌고찰이다. 농촌 보건의료체계, 근골격계 질환 역학, 농업 노동환경, 의료 접근성 등을 포함한 국내·외 문헌을 검색하여 농촌 지역 또는 농업인을 대상으로 한 총 14편의 연구를 분석하였다. 국내 연구에서는 반복적·고강도 작업, 비인체공학적 자세, 고령화가 통증 발생과 만성화의 핵심 요인으로 확인되었고, 국외 연구에서도 유사하게 요추·어깨·무릎 중심의 높은 통증 유병률이 보고되었다. 또한 농촌 지역은 전문 인력 부족, 중재치료 및 재활자원 제한, 장거리 이동과 경제적 부담 등으로 인해 진단과 치료 접근성이 낮아 통증 지속과 기능 저하가 쉽게 나타났다. 이러한 결과는 농촌 지역의 만성통증 문제를 해결하기 위해 작업환경 개선, 근골격계 예방 프로그램, 지역 의료 인프라 강화, 원격의료·이동진료와 같은 접근 성 보완 전략이 필요함을 시사한다. 본 연구는 농촌 지역의 통증 관리 격차를 구조적으로 이해하고 향후 보건정책 수립 의 근거 마련에 기여하고자 한다.
