This study utilized data from the 2022 Medical Service Experience Survey to extract 8,980 outpatient users, dividing them into groups with and without medical expense burdens. These groups were compared by their personal characteristics, and types of medical institution utilization, such as whether they had health check-ups, which outpatient services were utilized, medical institution satisfaction, and treatment goal achievement. The results of these findings then explored outpatient healthcare service use in relation to medical expense burden using chi-square analysis and t-tests. Out of the total 8,980 individuals, 493 (5.5%) belonged to the medical expense burden group. The medical expense burden group had a higher proportion of certain individuals compared to the non-burden group. These included women (p<.001), people aged 60 and above (p<.001), those with an educational level of elementary school or below (p<.001), low-income individuals (p<.001), beneficiaries of medical aid (p<.001), and those who did not undergo health check-ups (p<.001). This study compared outpatient services, satisfaction with medical institutions, and the achievement of treatment goals. It found that dissatisfaction with medical institutions was higher in the medical expense burden group (p<.001). This research highlights the characteristic of individuals who experience a higher healthcare expense burden and is expected to inform future policy development to help reduce this financial strain.

본 연구는 의료비 부담에 따른 외래 의료서비스 이용을 탐색하기 위해 2022년 의료서비스경험 조사자료를 활 용하여 외래 이용자 8980명을 추출하여 의료비 부담군과 비부담군으로 나누었다. 두 그룹에 대해 개인특성, 의료기관 이용 형태와 건강검진 수검유무, 외래이용서비스와 의료기관 만족도, 치료목표달성정도를 비교분석하였다. 분석은 카 이제곱분석과 t 검정을 실시하였다. 분석결과, 총 8980명중 493명(5.5%)이 의료비 부담군이었다. 개인특성중 여성 (p<.001), 60세 이상군(p<.001), 초등학교 교육수준이하(p<.001), 저소득층(p<.001), 의료급여자(p<.001), 건강검진 미수 검자(p<.001)의 비율이 의료비 비부담군에 비해 부담군에서 높았다. 외래서비스, 의료기관만족도와 치료목표 달성정도 점수를 비교한 결과 의료기관에 대한 불만족도 점수가 의료비 부담군에서 높았다(p<.001). 본 연구결과에서 의료비 부담군의 특성을 확인할 수 있었고, 본 연구를 활용하여 부담감 감소를 위한 정책개발연구가 추후 이루어질 것으로 기대한다.
