궁둥신경과 대퇴신경가동술이 만성 뇌졸중 환자의 엉덩관절 관절가동범위와 족저압에 미치는 영향
Effects of Femoral and Sciatic Nerve Mobilization on Range of Motion of Hip Joint and Plantar Pressure in Chronic Stroke Patients
This study was to investigate the effect of the nerve mobilization(femoral and sciatic nerve) on the affected leg on the improvement of the PP(Plantar Pressure) and ROM(Range Of Motion) in chronic stroke patients, in chronic stroke patients. The results of the study are as follows; The experimental group and the control group was a significant difference between pre and post intervention on the affected hip joint of the ROM after intervention(p<0.05). There was no statistically significant difference between the groups of the ROM before intervention, but there was a significant difference after intervention(flexion t=2.069, p=0.048, extension t=2.302, p=0.029, adduction t=2.078, p=0.047, abduction t=2.194, p=0.037). but The experimental group and the control group was no significant difference PP after intervention(t=-0.899, p=0.376). the results of the study suggest that if therapists conduct nerve mobilization based on the patient's condition and environment, they could improve chronic stroke patient's range of motion of the Hip joint

본 연구는 만성 뇌졸중환자의 마비측 다리에 신경가동술(넙다리신경과 궁둥신경)을 적용하여 마비측 엉덩 관절 가동범위와 족저압에 미치는 영향을 알아보고자 실시하였다. 연구 결과 마비측의 엉덩관절 가동범위와 족저 압은 실험군과 대조군 모두 유의한 차이가 있었다(p<0.05). 그리고, 마비측의 엉덩관절 가동범위 검사에서 실험군 과 대조군은 중재 전은 유의한 차이가 없으나 중재 후는 유의한 차이가 있었다(굽힘 t=2.069, p=0.048, 폄 t=2.302, p=0.029, 모음 t=2.078, p=0.047, 벌림 t=2.194, p=0.037). 하지만 족저압은 중재 전과 후의 비교에서 실험군과 대 조군의 유의한 차이가 없었다(t=-0.899, p=0.376). 본 연구 결과 등을 바탕으로 만성 뇌졸중 환자의 상태 및 환경 에 맞춰서 신경가동기법을 실시한다면 환자의 마비측 엉덩관절 가동범위 증가에 도움을 줄 수 있을 것이다.
