The purpose of this study is to identify the structure and archetype of typical conversations between visiting nurses and visiting nursing patients. For data collection, middle-aged and elderly people living alone and nurses among the subjects of visiting nursing at a public health center were the participants. After seeking consent for the study, the conversations were recorded, transcribed, and analyzed according to the conversation analysis methodology. As a result, the conversation between middle-aged and older adults living alone and nurses consisted of introduction, development, and final stages. It was confirmed that the characteristics of conversation progressed from comprehensive questions to specific questions, and nursing assessment and nursing intervention appeared repeatedly, and nurses' authoritative or accusatory conversations became communication obstacles. This study will be the basis for developing effective communication strategies by confirming the contents and types of communication between nurses and subjects.

본 연구는 방문간호사와 만성질환을 가진 방문간호 대상자 간의 전형적인 대화의 구조와 대화원형을 파 악하기 위한 것이다. 자료수집은 일개 보건소 방문간호 대상자 중 독거 중고령자와 간호사를 참여자로 하였으며 수집된 자료를 대화분석 방법론에 따라 분석하였다. 그 결과 독거 중고령자와 간호사간의 대화는 도입, 전개, 마무 리 단계로 구성되었다. 도입단계는 기본적인 인사와 안부 나눔 등으로 전개되었고 전개과정에서는 간호사정과 간 호중재가 중심적으로 일어나며, 간호사 발화의 대화 연속체는 6가지 유형, 대상자 발화 주도의 대화연속체는 4개 의 유형으로 분류되었다. 간호사와 독거 중고령자 간의 대화의 특징은 포괄적 질문에서 구체적 질문으로 진행되 고, 대화의 전개과정은 간호사정과 간호중재가 반복적으로 나타났으며, 간호사의 권위적이거나 비난적 대화는 의 사소통의 장애가 됨을 확인하였다. 본 연구는 간호사와 대상자 간의 의사소통의 내용과 대화유형을 확인함으로써 챗봇 개발이나 효과적인 의사소통 전략 개발에 기초가 될 것이다.
