Objective : The purpose of this study is to identify predictive factors that affect depression in stroke patients, and to provide basic data on the need to approach the occupational therapy intervention program in stroke patients by considering psychological factors such as alleviation of depression. Method : In order to identify predictive factors affecting depression in stroke patients, a secondary analysis was conducted on 91 stroke patients using data from the second year of the 8th National Health and Nutrition Examination Survey. We analyzed general characteristics, health-related characteristics, and stroke-related characteristics were in order to identify predictors of stroke patients. Depression was measured using the Korean version of the Depression Screening Tool(PHQ-9). Result : The prevalence of depression among stroke patients was 11.6%. Among the characteristics of stroke patients, there was a significant difference in marital status among general characteristics, activity restriction, perception of usual stress, and limitation of daily living activities among health-related characteristics(p<0.05). There was no significant difference in variables of stroke-related characteristics (p>0.05). As variables affecting depression in stroke patients, it was found that depression increased when there was activity restriction(B=3.719), in a married state living alone(B=2.895), and when there was a lot of stress(B=2.824)(p<0.05), and the explanatory power of the regression analysis was 33.7%. Conclusion : In order to maximize the therapeutic effect in establishing occupational therapy intervention strategies for stroke patients with depression, it is thought that it is necessary to add programs for reducing activity restriction, increasing social support, and reducing stress, which are predictive factors for depression in stroke patients.

목적: 본 연구는 뇌졸중 환자의 우울증에 미치는 예측요인을 확인하여, 뇌졸중 환자의 작업치료 중재 프로그램에 있어 우울증과 같은 심리적인 요소를 고려하여 접근할 필요성에 대한 기초자료를 제공하는데 목적이 있다. 연구방법: 뇌졸중 환자의 우울증에 영향을 미치는 예측요인들을 확인하기 위하여, 국민건강영양조사 제8기 2차년도 자 료를 활용하여 뇌졸중 환자 91명을 대상으로 2차 분석을 실시하였다. 뇌졸중 환자의 예측요인을 확인하기 위하여 일반적 특성, 건강관련 특성, 뇌졸중 관련 특성을 분석하였다. 우울증은 한글판 우울증선별도구(PHQ-9)로 측정하 였다. 결과: 뇌졸중 환자 중 우울증 유병율은 11.6%였다. 뇌졸중 환자의 특성 중 우울증의 유의한 차이가 있는 변수는 일반적 특성 중 결혼상태, 건강관련 특성 중 활동제한, 평소 스트레스 인지, 일상생활활동 제한이 유의한 차이가 있었으며 (p<0.05), 뇌졸중 특성 관련 변수는 유의한 차이가 없었다(p>0.05). 뇌졸중 환자의 우울증에 영향을 미치는 변수로 활동제한이 있는 경우(B=3.719), 혼자 사는 결혼 상태인 경우(B=2.895), 스트레스를 많이 받는 경우(B=2.824) 우울증 이 증가하는 것으로 나타났으며(p<0.05), 회귀분석의 설명력은 33.7%였다. 결론: 우울증이 있는 뇌졸중 환자의 작업치료 중재 전략 수립에 있어 치료 효과를 극대화하기 위해서는 뇌졸중 환자의 우울증 예측요인인 활동제한의 감소, 사회적지지의 증가, 스트레스 감소 위한 프로그램을 추가할 필요성이 있을 것 으로 사료된다.
