Sudden natural death is frequently described as 'rapid', 'unexpected' or 'natural death from an internal cause'. Sudden cardiac death, the most common type of sudden death related to cardiovascular or coronary artery disease, cannot be determined as the cause of death by a postmortem examination alone. It is difficult to exclude the possibility of external factors or unnatural death when the exact circumstances surrounding the death and the nature of external injuries are not well established. All seven cases in this study involved sudden cardiac death, but in two of the seven cases an autopsy was performed due to the presence of external injuries and the possibility that the deaths were not 'natural'. Other cases were identified at postmortem along with an investigation of past medical history and consideration of the location where the death occurred. Additional information, such as life history, psychiatric data, and other details concerning clinical history, as well as scene investigations, are normally required to confirm sudden cardiac death.

내인성 급사란 ‘내인사(natural death)’와 ‘급사(sudden death)’를 합한 표현으로 내적 원인에 의한 죽음을 말한다. 내인성 급사 중 급성심장사는 대부분 심장 관상동맥질환에 의하여 사망하게 되는데 현장이나 영안실에서 외표 검시만으로는 사인을 가릴 수 없는 경우가 빈번하게 발생한다. 내인성 급사로 사망한 경우라 하더라도 외상이 동반되어 있고 죽음의 경과가 명백하지 못하면 외인사 또는 외인의 작용 가능성을 배제할 수 없게 된다. 본 논문에서, 두 사례는 급성심장사로 사망하였으나 외상이 발생하여 검시 소견만으로는 내인사라는 결론을 내리기 어려웠던 사례로 부검 의뢰하였으며, 다른 사례들은 변사자 검시와변사자 과거병력 조사, 현장상황 등을 조사하여 타살의 가능성을 배제하고 내인사 즉 급성심장사로 사건종결한 사례들이다. 급성심장사라는 사인을 판단하기 위해서는 사망자의 병력과 평소 생활 습관, 사망상황에 대한 법의학적 조사, 현장 검안 및 현장 조사가 필요하다.
