The Medicaid case management system has been implanted to improve Medicaid beneficiary health-related quality of life and efficiency of the Medicaid finance since 2003 in Korea. Emphasis is given to the recipient of reasonable utilization of medical care. In order to provide health and welfare care through the Medicaid case management program, case managers, whose total number is approximately 480 nurses as of December 2010, have been working at 244 local government agencies across the country. Their role in the program includes not only providing information guide, psychological support, health education and counseling, but also networking the related health and welfare resources through personal home-visits, telephone, and mailing. The author reviewed articles and reports related effects of the Medicaid case management system. The case management approach was shown to be effective to improve the beneficiaries’ health-related quality of life and to decrease the cost of the medical treatment on outpatient clinics. The author proposes four policy directions as follows: building an integrated circulation system for the case management; development of standardized instruments and clinical guideline by the type of recipients; development of resources network system; development of integrated information system.

우리나라 의료급여사례관리는 2003년 5월부터 의료급여 1종 수급권자를 대상으로 시작되었으며, 그들의 삶의질 향상과 합리적 의료이용을 유도하기 위함이었고, 궁극적으로 의료급여 재정 안정화를 도모하기 위함이었다. 2010년 현재 전국 244개 보장기관에 약 480여명의 의료급여관리사가 배치되어 의료급여수급권자 중에서 신규로 의료급여수급권자가 된 자, 병의원의 외래를 과다 또는 과소 이용하는 자, (요양)병원에 장기적으로 입원해 있는 자들을 대상으로 사례관리를 수행하고 있다. 의료급여관리사들은 전화, 방문, 서신 방법 등을 활용하여 정보제공, 심리적 지지, 보건교육 및 상담, 연계 및 의뢰 서비스를 제공한다. 본 연구는 문헌고찰을 통하여 의료 급여사례관리가 의료급여수급권자의 건강 삶의 질, 자가 관리 능력, 합리적 의료이용, 지지체계 영역에서의 향상과 외래 진료비 감소에 효과가 있는 반면에 입원 진료비는 증가하였음을 알 수 있었다. 이를 기반으로 효율적인 사례관리를 위한 발전방향을 첫째 통합순환 사례관리 체계구축, 둘째 표준화된 임상지침 및 사정도구개발, 셋째 자원 네트워크체계 구축, 넷째 통합 정보체계 개발로 제시하였다.
