Health security for the elderly who are hard to maintain minimum livelihood is basically in the boundary of the national basic livelihood security system. Health security for the elderly in the national basic livelihood security system includes medical assistance, health examination for the elderly, and eye examination or operation for gaining eyesight. Long-term care service is provided under the system of long-term care insurance program. In order to improve health security for the elderly in the national basic livelihood security system, extent and length of medical assistance must be extended. The function of public health centers in rural areas have to be also strengthened because most of medical institutions are located in urban areas. The government should take responsibility for giving health examination for the elderly in the national basic livelihood security system through revision of the related law since it gives the health examination at its discretion by law. The items of the health examination need to be added.
